Research Connect CTR Inc. Consent Form

Term

This is an Agreement between you (hereinafter referred to as “you” or the “User”) and Research Connect CTR Inc. (hereinafter referred to as “We”, “Us, or the “Site”) and governs your use of Research Connect CTR Inc. website, its content (“Website) and Research Connect CTR Inc. platform (collectively, the “Services”) in relation to collecting the User’s information including personal health information (PHI) to identify a User as a likely candidate to participant in a current or future clinical research trial study. Once the User is identified as an appropriate candidate by the Site, We will then inform the User of their eligibility. Once the User is informed as a likely candidate by Us, the User will be solely responsible for registering and applying to the suitable clinical trial directly. We will not be responsible for contacting any third-party clinical research trial for the User, nor will we disclose information provided by the User to any third-party clinical research trial.

For the purpose of this Agreement, Non-Clinical Research Services are defined as any Research Connect CTR Inc. product or service available via the Website or Mobile Application. This Agreement is legally binding you and Research Connect CTR Inc.

BY USING THE SERVICES, YOU ARE AGREEING THAT YOU ARE AT LEAST THE AGE OF MAJORITY IN PROVINCE OR TERRITORY OF RESIDENCE AND YOU ARE LEGALLY ABLE TO ENTER INTO A CONTRACT. If you are under the age of sixteen, you are required to provide the designated substitute decision-makers’ express consent to use the Site Services. A designated substitute decision-maker can be your parent, Children’s Aid Society or another person who is legally entitled to provide consent on your behalf to the collection, use or disclosure of your PHI, except in certain circumstances (such as receiving medical treatment).

The Website and the Services may not be used by persons in jurisdictions where the access to or use of the Website and the Services or any part may be illegal or prohibited.

ResearchConnect will use your provided data to identify the studies that may be best suited for you based on your conditions, location, race and ethnicity. By submitting your information, you agree to our Privacy Policy and Term of Use and our processing of the information provided to fulfill your request for our services, including notifying you of customized study recommendations that are or become available, and to (our sharing of
your responses with the clinical trial doctor’s office or match you to a clinical study for you to contact and register to participate as a research candidate directly). You agree to indemnify ResearchConnect and its employees, directors, officers, custodians and agents for any liability which it or any of them may incur, arising out of the inspection, copy or use of the information you provided on this form.

You agree to provide your written consent to provide the following information to us:

  • Name and contact information
  • Date of birth or age
  • Health history and current medical conditions
  • Medications or treatments you are receiving
  • You also acknowledge that you understood our services. You also acknowledge that you understood the individual clinical research studies’ structures, benefits, any risk of harm and how your confidentiality will be handled by these clinics that you may directly register and participate with that our applicable and suitable to you.
 

You understand the PHI We collect from You will be handled and used according to Personal Health Information Protection Act, 2004, (PHIPA) and Personal Information Protection and Electronic Documents Act, 2000, (PIPEDA and other applicable laws. Should We be required to disclose to a third party that is not a health professional or disclosure to a health profession that does not pertain to an individual’s health care services we will require your express consent to do so.

You can withdraw your consent to use your PHI at any time by sending an email directly to researchconnectctr@outlook.com. Please provide the date you want to effectively withdraw your consent and reasons to do so in your email request.
